ITS Service Requests
Information Technology Services (ITS) offers services to the FGCU community to support the needs of the University. Faculty, staff and students wishing to request core ITS services should use the online forms below. This collection of forms is created to gather necessary information specific to the service being requested to provide a more timely response.

Accounts and Access

Access Request
All Access Request should be submitted by a department’s designated Authorized Account Requestor (AAR)
The "Access Request" form is intended to be used for the following purposes:
1. Adding or removing an employee to an existing resource
2. Adding or removing multiple employees to an existing resource; select the "Multiple accounts" option.New Resource Request
New Resource Request: Shared mailbox, Shared folder should be submitted by a department’s assigned Authorized Account Requestor (AAR)
The "New Resource Request" form is intended to be used for the following purposes:

Project Request
A project request to Information Technology Services (ITS) is the first step in initiating a “technology as a service” endeavor, that has a defined scope and is conducted within a specific timeline to accomplish a specific goal, such as to solve a business problem.

Software/Hardware Purchase Request
This process is now included in the purchase process in Workday and will automatically be completed.
Request ServiceSoftware Install Request
To request software installation on your office or any classroom/lab computer. An approved Software/Hardware Purchase Request must be on file before software can be installed.

LTI Request
Learning Tools Interoperability (LTI) is an external piece of software plugged into FGCU's learning management system (Canvas) to extend its functionality.
Request ServiceFaculty Secure PC Opt-Out
Secure PC ensures that computers stay up to date and secure by centrally managing installed software applications. This form will allow any faculty member to request an exemption from Secure PC.
Request ServiceFormstack Request
Formstack is a platform that allows the creation and management of online forms. This service will allow any faculty and staff member to request the implementation of a new form, along with the creation of support tickets regarding Formstack accounts, existing forms, and workflows.

REQUEST SERVICEComputer Move
This form is used to request any type IT Move, ranging from workstations moving within the same department or office, to a series of moves involving multiple departments and buildings.
REQUEST SERVICENetwork Printer Setup
This form is used to request the setup of an approved university owned printer that will be connected to the FGCU network.
Network Port Activation
This form is used to request the activation of a network port.

Change Management & Training

Most ITS projects involve change, requiring some level of training and communication using one or more delivery methods, which may include: classroom training, open lab, on-demand video, User Manual or Job Aid, etc. with needed communications woven throughout the initiative.
